What is risk-reducing surgery?
- Definition
- Risk-reducing (prophylactic) mastectomy is preventive surgery that removes most breast tissue to lower the chance of developing breast cancer. It is offered to people at high genetic or familial risk. It is usually combined with reconstruction performed at the same time.
Who might consider it
Risk-reducing surgery may be appropriate if you have:
- A confirmed BRCA1, BRCA2, PALB2, TP53, CHEK2 or PTEN mutation
- A strong family history of breast or ovarian cancer pending genetic testing
- A previous diagnosis that raises your risk in the other breast

Risks and considerations
This is major, irreversible surgery undertaken when you are well, so the decision deserves careful thought. Risks include those of any breast surgery and reconstruction — infection, bleeding, changes in sensation and the possibility of revision. Surgery greatly reduces but does not entirely remove future risk. We support you throughout, in line with NICE familial breast cancer guidance.

Who is eligible for risk-reducing surgery?
It is considered for people with a confirmed high-risk gene mutation (such as BRCA1, BRCA2, PALB2, TP53, CHEK2 or PTEN) or a strong family history under genetic assessment. The decision is highly personal and made with genetic counselling.
How much does it reduce my risk?
Risk-reducing mastectomy substantially lowers the lifetime risk of breast cancer — by around 90% or more — though no operation removes risk entirely. Your individual figures are discussed during assessment.
Can I have reconstruction at the same time?
Yes. Mr Ghosh performs risk-reducing mastectomy with same-stage reconstruction in most cases, so you wake with a reconstructed breast shape.
Do I have to decide quickly?
No. This is preventive surgery and there is no urgency. We encourage you to take the time you need, supported by genetic counsellors and the wider team.
What are the alternatives to surgery?
Enhanced surveillance with regular MRI and mammography, and risk-reducing medication, are alternatives. We will explore all options so your decision is informed.
